Sauna use was associated with lower mortality—not proven to extend life
One Finnish male cohort linked frequent sauna use with lower mortality. The design cannot show that sauna caused longer life, and randomized evidence has not tested that endpoint.
A striking association with a strict causal boundary
Over a median 20.7 years, men reporting more frequent sauna use had lower rates of several mortality outcomes. That is useful observational evidence, but it is not a randomized test of whether sauna extends life.

The bottom line
Four to seven sauna sessions per week were associated with lower sudden-cardiac-death risk than one weekly session in this cohort. Because participants chose their own habits and exposure was recorded only at baseline, the result cannot establish cause and effect.
What researchers did
Researchers followed 2,315 men from eastern Finland who were 42 to 60 years old at baseline. Sauna frequency groups contained 601 men using sauna once weekly, 1,513 using it two or three times weekly and 201 using it four to seven times weekly.
What they found
During median 20.7-year follow-up, the cohort recorded 190 sudden cardiac deaths, 281 fatal coronary heart disease events, 407 fatal cardiovascular disease events and 929 deaths from any cause. For sudden cardiac death, the adjusted hazard ratio for four to seven versus one weekly session was 0.37 (95% CI 0.18–0.75).
How strong is the evidence?
The long follow-up and hard outcomes are strengths. The observational design, one-time self-report, small highest-frequency group and possible differences in health, wealth, activity and other habits limit causal inference. A 2025 review of 20 passive-heating randomized trials did not find pooled effects for most short-term surrogate outcomes and did not test mortality.
What this does not show
It does not show that sauna caused the mortality difference, identify an optimal exposure, establish benefit for women or other populations, or prove prevention of cardiovascular disease. An association with mortality is not a lifespan intervention trial.
Safety, guidance and conflicts
Heat exposure can contribute to heat illness, dehydration and fainting and may be inappropriate for some people. The cohort reported public and foundation support and no conflicts; residual confounding remains the central limitation.
What happens next
A causal answer would require well-controlled trials with clinically meaningful outcomes, adequate follow-up and populations broader than middle-aged Finnish men. Short-term changes in blood pressure or vascular markers cannot substitute for mortality evidence.
